What is social prescribing?
Social prescribing is an NHS model that helps connect you with the community to manage your health and wellbeing. There are lots of people, activities, groups and services in the community that can help meet your needs. For example, volunteering roles, cooking classes, sports and more.
Getting a social prescribing link worker
There are local agencies, such as local charities, social care and health services that can refer you to a social prescribing link worker. They can give you support and connect you with what’s going on in the community to help you take control of your mental health and wellbeing.

Live well B&NES Bath & North East Somerset's community website provides information, support and signposting for families, children and young people with additional needs and adults needing care or support.
Available 24/7 (online) Live Well B&NES supports:
- Adults needing care and support to live independently

- Parent Carers of young people with Special Educational Needs and Disabilities
- Young People with additional needs
- Young People looking for universal information to promote their mental health, opportunities and engagement
